Step 2: You will see two enrollment options, paper or online. For online registration, click the red box marked, “I want to check my county.”
To download and print a paper form, click the white box marked “I will submit my own request form.”

Step 3: Be prepared: You will need to provide the last four digits of your social security number OR your Florida Driver’s License/ID number to complete the request.
